I live in Florida, a right to work state who consistently elects Republican governors. I am, frankly, feeling a very large dread upon me (as my mom would have said). Otherwise all the 'right to work' States will automatically opt out and to continue to have the same 25% of their population uninsured and most of those insured will continue to be insured only by a small cartel of price fixing insurers. Those States that support business interests over those of their constituents will always have a good lie to opt out of 'providing for the public welfare.' If it is important enough to have federal legislation, then it is important enough for everyone to have a vote on it in their own State.
